Introduction: As female-headed households belonged to high risk group in population therefore they experience more stress and anxiety, and also risk of mental disorders. Hence the present study aims determine of association of mental Health and quality of life in Female-headed households under cover Welfare Organization of Ilam Province.
 
Materials & Methods: Analytical cross-sectional study ,and The study population included all women heads of households under the auspices of the Welfare Organization of Ilam Province. Systematic random sampling used to as a sampling method.  Valid questionnaires including (GHQ - 28) questionnaire to assess mental health status and (SF - 36) questionnaire were used to assess participantschr('39') quality of life , The results were analyzed using SPSS statistical software version 21 using t-test. Significance level in this study was considered α<0/05
 
Findings: From 750 Female-headed households contributing to the project, 708 completed questionnaires (response rate 94%). Mean ± SD age of participants was 53.3± 19.18. According to the results, a total average life quality score was 46.08 and 79 % of Female-headed households had a problem in terms of general health. A woman’s with better general health in this study had a better quality of life according to results (p= <0.001).
 
Discussion & Conclusions: Results of this study an emphasis on low quality of life of in women heads of households and their association with various aspects of general and mental health in this group. Women with lower quality of life showed more physical and mental problems.
